Listed below are the forms and documents required for application in the Alabama Trauma and Health System (ATHS).

Out-of-state hospitals requesting to be a designated hospital in the ATHS will follow the same process as outlined in 420-2-2-.03. Out-of-state hospitals will be required to meet the same standards as Alabama hospitals and follow the same survey and reporting processes.
