CCD State Plan

The Coordinated Chronic Disease (CCD) Prevention Program has expanded its capacity to address chronic diseases through collaboration across different programs of the Alabama Department of Public Health: Cardiovascular Health, Diabetes, Asthma, Arthritis, Cancer, Tobacco Control, and Nutrition and Physical Activity, as well as with the coalitions and organizations working with these programs. The Coordinated Chronic Disease Prevention and Health Promotion state plan is part of the process to integrate the work across the different chronic disease programs to maximize the impact of all the programs.
