This sheet provides information about CDSMP and explains its importance.

This sheet provides evidence-based information about CDSMP. It also outlines counties in Alabama they are covered with CDSMP and organizations offering CDSMP.
- Workshop Information Flyer (for public use)
This flyer can be posted on bulletin boards in public places to inform the community of an upcoming workshop.
- Workshop Information Flyer (for individual use)
This flyer is more specific as to what the program covers and can be given to individuals who will potentially participate in the program.
- Group Leader Script
Lay Leaders must follow this script to inform participants of the uses of the participant information survey and to explain the consent form. This should be done at the beginning of the Session One workshop.

This survey should be completed by each willing participant and returned by the Lay Leaders to Living Well Alabama within 15 days of workshop completion.

This form should be completed by each participant and returned by the Lay Leaders to Living Well Alabama within 15 days of workshop completion.
- Workshop Information Cover Sheet
This form must be completed by Lay Leaders and returned to Living Well Alabama within 15 days of workshop completion.

This form must be completed to log the attendance of workshops and returned to Living Well Alabama within 15 days of workshop completion.
- Non-disclosure Agreement
Each lay leader should sign and return this agreement prior to beginning a workshop. Effective: September 1, 2013

This resource provides an outline of what will be covered in the sessions and includes homework for each session.
There are charts available for every session. They can be printed or handwritten. This file is set up for 24x36 inch printing.

This is a blank action plan form that can be used in workshops that require action planning.
- Completer Certificate
This fillable certificate should be given to each participant who has attended 4-6 sessions upon completion of CDSMP.
