Overdose Notification

breakthecycle.jpgThe Alabama Department of Public Health (ADPH) is creating a statewide system to notify stakeholders in the event of significant drug overdose events. Any individual residing in the state can submit their information to be notified when these drug overdose events occur. Notifications will be sent by email and text message.

Using overdose data reported through a statewide monitoring system, ADPH will identify drug overdose events that warrant stakeholder notification. All overdoses will not result in notifications being sent. Instead, notification will be based on an algorithm that considers an unusual occurrence or pattern of suspected overdoses for further response.

By submitting the information using the form below, you agree to be contacted by the Alabama Department of Public Health for the purposes of healthcare and related services and administration.

The information you provide will be retained by ADPH for the sole purpose of providing you with information and updates regarding the occurrence of drug overdose events in your area.

The information you provide is solely for the use of ADPH as a form of communication for notification of drug overdose events and will not be used for any other purpose. Your information will not be shared with other individuals or organizations under any circumstances.

Funding for the Alabama Overdose Response Project was provided by the ADPH, Bureau of Prevention, Promotion, and Support through a cooperative agreement with the Centers for Disease Control and Prevention (CDC), being Grant number 6 NU17CE924964-02-02, Overdose Data to Action, Injury Prevention and Control Research and State and Community Based Programs Funding (93.136) for budget period September 1, 2020 through August 31, 2021.
