Home Care Program

Health care services for residents of Pike County who meet eligibility requirements are provided through the Crenshaw County Health Department. We provide home health care services prescribed by a physician to residents of Pike County. Services are delivered to qualified homebound Medicare, Medicaid, or commercial insurance recipients.

A home health team of highly trained professional staff provide care on an intermittent part-time basis in the privacy of the home to acutely and/or chronically ill individuals. Services provided may include:

  • Skilled Nursing
  • Physical Therapy
  • Medical Social Services
  • Home Health Aide

Our staff can provide disease management of many conditions and symptoms that were previously limited to the hospital setting, to include:

  • Post-surgery Care
  • Heart Failure
  • COPD
  • Diabetes Mellitus
  • Hypertension
  • Stroke
  • Wound Care
  • Pediatric Care
  • Arthritis
  • Alzheimers
  • Muscle Skeletal Conditions
  • Medication Teaching/Management

Office hours are 8:00 a.m. to 5:00 p.m. Monday through Friday, excluding holidays. Skilled Nursing Services are available 24 hours a day, seven days a week, including holidays. (334)-335-2532 or 1-855-633-5015.

Eligibility

Eligibility varies by the insurance and reimbursement requirements, and by each individual's specific medical needs. A physician order is required for all services.
