Vital Records

To apply for Alabama birth, death, marriage, or divorce certificates you may print the application request form below and take it to any local county health department in Alabama, or mail a completed application request form and one form of identification from the Primary ID list, along with the appropriate fee to:

Application for a Birth, Death, Marriage, or Divorce Certificate

Customers can obtain only Alabama vital records from the Center for Health Statistics through their county health department in 30-45 minutes or less to locate the record. Should the record require a manual search (older records that have not been scanned into the database, or records that are resubmitted due to incorrect information on the application), the search may take longer than 45 minutes. Applications are accepted on Monday through Friday, except State Holidays (or unanticipated closings like weather or system outages), between 8:00 a.m. until 4:15 p.m. Records may also be obtained through the mail, in about 7-10 days. The cost for one (1) certified certificate is $15.00 for the first copy, and $6.00 for each additional certified copy. The Madison County Health Department accepts cash, checks, with a Valid Alabama ID, and credit cards, unless system outages occur.
