Mass Vaccination Clinics

As part of a statewide effort to accelerate COVID-19 vaccination in Alabama, vaccination clinics will be held in each of the state’s public health districts. There is no cost to receive a vaccine. Everyone must wear a mask and bring valid identification, and an insurance card if you have one.

Important Notices:

  • Persons receiving vaccine at the clinics listed below will need to complete a consent form and review five forms provided under the Vaccination Forms section at the bottom of this web page prior to receiving their vaccine. To expedite the vaccination process, persons may complete the consent form found on the website ahead of time and bring it to the vaccination site. For those without the ability to print the forms, materials will also be available at the vaccination locations.
  • Please check this page frequently as new locations will be added and existing locations are subject to change due to inclement weather or unforeseen logistical issues.

Monday – Thursday, 9:00 a.m. to 5:00 p.m. and Friday from 10:00 a.m. to 3:00 p.m.
Location: Birmingham-Shuttlesworth International Airport (Near the airport terminal off Aviation Avenue at the end of 50th Street North & 18th Alley across from Kaiser Aircraft Industries)

Please complete, print, and bring the following form with you to the vaccination clinic to shorten the process. If you do not have your form with you when you arrive, you will be able to review and complete the form onsite.

We ask that you review these five forms before you receive your first dose of the Pfizer vaccine.
