Home Care Program

The Barbour County Health Department Home Care Program provides home health care services prescribed by a physician to residents of Barbour County. Services are delivered to qualified homebound Medicare, Medicaid, or commercial insurance recipients.

A home health team of highly trained professional staff provides care on an intermittent part-time basis in the privacy of the home to acutely and/or chronically ill individuals. Services provided may include:

• Skilled Nursing
• Physical Therapy
• Medical Social Services
• Home Health Aide

Our staff can provide disease management of many conditions and symptoms that were previously limited to the hospital setting, including:

• Post-surgery Care
• Heart Failure
• COPD
• Diabetes Mellitus
• Hypertension
• Stroke
• Wound Care
• Pediatric Care
• Arthritis
• Alzheimer's
• Muscle Skeletal Conditions
• Medication Teaching/Management

Eligibility

Eligibility varies according to insurance and reimbursement requirements and each individual's specific medical needs. A physician order is required for all services.
