Healthy Heart Ambassador

The Healthy Heart Ambassador Blood Pressure Self-Monitoring Program is a 4-month program that helps people with hypertension — also known as high blood pressure — learn how to accurately take their blood pressure at home.

Participants will:

  • Get four months of personalized support from a Healthy Heart Ambassador.
  • Learn how to properly take and record their blood pressure at home.
  • Meet individually with a Healthy Heart Ambassador twice a month.
  • Measure and track their blood pressure at least twice a month.
  • Attend monthly group nutrition education seminars.

Who can join?

Adults (18 years or older) in Dallas, Houston, and Montgomery counties who meet the following criteria:

  • Diagnosed with high blood pressure by a health care provider and/or on blood pressure medicine.
  • Not had a heart attack or stroke in the last year.
  • Not have atrial fibrillation, heart arrhythmias, or lymphedema

Cost

It is free to join for participants who qualify to participate in this program.
