Anonymous Complaints

The Alabama Department of Public Health (ADPH) has established a Compliance program in accordance with federal, state and local regulatory requirements. All ADPH employees are required to report wrongdoing, suspected wrongdoing, or any violation of ADPH policies and procedures relating to compliance issues.

This is not for environmental complaints. If you wish to file a complaint on food facilities and foodborne illness, hotel/lodging, body art (tattoo), and other environmental issues under the jurisdiction of ADPH, please visit Environmental Complaints.
