Home Care Services

Home Health Program

The Conecuh County Health Department provides home health services prescribed by a physician to residents of Butler, Conecuh, and Covington Counties. Services are delivered to qualified home-bound Medicare, Medicaid, and other commercial insurance recipients.

Our home health program allows the patient the opportunity to receive physician prescribed skilled care in the comfort of their own home. Below is a list of services that are provided to patients under our Home Health Program.

  • Skilled Nursing Services: Licensed, trained Registered Nurses provide complete physical assessments, wound care, central line and g-tube maintenance, IV therapy, medication administration, and other nursing services.
  • Home Health Aides: Certified Home Health Aides care for our patients' personal hygienic needs, change linens, perform light laundry duties, and assist with preparing small meals.
  • Medical Social Services: Licensed medical social workers assist our patients in coping with their social, economic, and emotional needs by helping them gain access to and utilize available community services and resources.
  • Physical Therapy: Registered Physical Therapists and Licensed Physical Therapist Assistants work to improve mobility and increase strength and endurance. They also provide treatment to relieve pain.
