Social Work Services

Social workers from the ADPH Southwestern District provide the following case management services to the residents of Conecuh County:

• Care Coordination for Newborn Screening Program
• Care Coordination for Children with Elevated Lead Levels
• Outreach Coordination for Diabetes and Cardiovascular Programs
• DHR/Early Head Start Care Coordination (Program serves Mobile, Conecuh, and Washington Counties)
• HIV Re-Engagement Program (HREP)
