We are pleased to inform you that the application for initial licensure for Massage Therapists/Massage Therapy Establishments is available from the LMT Massage Therapists Page and the Establishments Page. The application requires several items, so be sure to review the instructions before you apply and the Initial Application Package, and follow them closely to ensure all information is included and your application is not delayed.
Renewal applications for Licensed Massage Therapists and Massage Therapy Establishments are available from the LMT Massage Therapists Page and the Establishments Page. Each application includes a supplemental package, which outlines documentation and other requirements. Please print or download the supplemental package and follow the instructions carefully to avoid processing delays.
Documentation for CE courses must be saved to your device and uploaded during the renewal process.

You may obtain a copy of your criminal history background check by filing the ALEA CJIS Form 46 Rev. ADPH v1 with ALEA. Please follow the instructions on that form. Do not submit the form to ADPH's Massage Therapy Program if you wish to obtain the criminal history for yourself.
To request a duplicate license, complete our Contact Form. You will need to provide the mailing or email address of the state licensing board to which your license verification should be sent. The fee for a license reprint/duplicate copy is $30.

Fees
- Massage Therapist List - Annual roster of active massage therapists - $50
- License Verification - $35
Appropriate legal documents must be submitted before the name is changed. You may submit any one of the following:
- Marriage certificate.
- Divorce decree substantiating the name change.
- Probate court records affecting a legal name change.
ADPH's Massage Therapy Program will waive the new establishment license fee provided that the holder of the massage therapy establishment license provides the following documentation:
(a) Proof of the massage therapy establishment's right to occupy the new premises as demonstrated by a deed, lease, or other document establishing lawful possession in the name of the sole proprietor, corporation, limited liability company, or partnership applying for a massage therapy establishment license.
(b) Proof that the licensed massage therapy establishment has vacated the prior location or is required to vacate the prior location by a date certain within sixty (60) days of the request.
The expiration date of the massage therapy establishment license will not change.
