New License - Instructions Before You Apply

These instructions are designed to assist you in the online application process.

Fees associated with the online application and criminal background check:

• Application and license fee for a two-year license: $150
• Actual cost of federal and state background checks: to be billed by a third-party vendor

Files to be uploaded to the application are marked with an asterisk (*) below:

• U.S. Citizenship or Lawful Presence: All applicants for licensure in the state of Alabama are required by law to provide proof of U.S. citizenship or lawful presence in the United States. Use the form that applies to you.
• Proof of current professional and general liability insurance with an "A" rated or better insurance carrier in the amount of at least one million dollars.
• Complete a minimum of 650 hours of instruction in massage therapy that satisfies the requirements of the Alabama Massage Therapy Licensing Act. Have the transcript emailed to [email protected].
• Pass the MBLEx or a national standardized examination approved by ADPH. Have the score report sent to [email protected]. Please note: Alabama Department of Public Health's Massage Therapy Program may approve state exams on a case-by-case basis. Currently, ADPH is not accepting Texas exams in lieu of a national standardized examination.
• License Verification from each state of licensure (even if expired). Email to [email protected].

License Renewal - Instructions Before You Apply

These instructions are designed to assist you in the online application process.

Note: A massage therapist license that has expired for more than five years may not be reinstated.

All fees are included in the online renewal application and processed online. Fees associated with the online application and criminal background check:

• Application and license renewal fee for a two-year license: $125
• Actual cost of federal and state background checks: to be billed by a third-party vendor

Files to be uploaded to the application are marked with an asterisk (*) below:

• U.S. Citizenship or Lawful Presence: All applicants for licensure in the state of Alabama are required by law to provide proof of U.S. citizenship or lawful presence in the United States. Use the form that applies to you.
• Proof of current professional and general liability insurance with an "A" rated or better insurance carrier in the amount of at least one million dollars.
• Sixteen (16) hours of Continuing Education.

Note: You must have an application on file with the Alabama Department of Public Health's (ADPH) Massage Therapy Program prior to submitting for fingerprinting. You cannot begin the background check process until AFTER the license application submission date. If ADPH's Massage Therapy Program receives a background check before receiving an application, the applicant will be required to undergo another background check. This will result in you having to pay for the background check again.
