Licensing

The State of Alabama requires that anyone who operates as or advertises as a massage therapist, or offers massage therapy services, be licensed by the Alabama Department of Public Health (ADPH). All businesses or establishments that offer massage therapy services must be licensed unless they are specifically exempt.

Fingerprinting

Alabama law requires a fingerprint criminal background check for each massage therapy applicant who has not completed an application since June 1, 2024, when the statute creating the Alabama Massage Therapy Licensing Board (AMTLB) took effect. If you have completed a fingerprint background check and maintained an active licensure status since June 1, 2024, you are not required to complete another.

Duplicate Licenses

To request a duplicate license, complete our Contact Form. You will need to provide the mailing or email address of the state licensing board to which your license verification should be sent. The fee for a license reprint/duplicate copy is $30.
