Who Should Take the Survey?

  • Alabama residents, especially women of childbearing age and individuals who are raising infants, children, and youth.
  • Teens/Adolescents your voice is also important and we want to hear from you.
  • Healthcare providers who serve the MCH population are also encouraged to complete the survey.

  • Confidentiality: All responses are anonymous and will be kept confidential to the extent allowed by law.

Background

The Alabama Department of Public Health, Office of Maternal and Child Health (MCH) is working with a team from the University of Alabama at Birmingham School of Public Health to conduct a five-year needs assessment on the health of women of childbearing age, infants, children, and youth (including children and youth with special health care needs) in Alabama. As part of the federal Title V MCH Services Block Grant, all jurisdictions complete a statewide, comprehensive needs assessment every five years to identify priority health issues, concerns, and areas of importance within MCH target populations. The five-year MCH Needs Assessment includes four main data collection components: an electronic survey, focus groups, key informant interviews, and listening sessions.
