Counseling and Testing Form

The Counseling and Testing form is used to collect pre-test and post-test information on clients receiving testing from providers who are using CDC funded test kits. If you are a CLIA-certified HIV/AIDS medical care provider funded for testing under CDC’s grant, you can request access to HADIS through the ADPH Security Portal. Go to Create an Account and follow instructions.

The old paper HIV Counseling and Testing forms (CL-109 Serology form/Addendum) are no longer being used as data collection templates. An updated data collection template has been created to include all required/updated variables that must be submitted when using the CDC-funded test kits provided by ADPH. The updated data collection template can be used to collect the required data. Once the data is collected, the information should be entered directly into HADIS or your agency’s internal system. You do not have to use this form unless it is helpful to you. You have the option of entering the data directly into HADIS or entering all required data into your agency’s internal system. If you enter the data into your agency’s system, you will need to provide ADPH with an XML file containing all required data. Please contact us for more information.

Please make sure to answer all required questions completely, including post-test questions for both HIV negative and HIV positive (including preliminary positive) clients. Please make sure to enter all data collected into HADIS or your agency’s internal system. Do not send paper forms back to ADPH. Paper copies of the updated data collection template can be ordered by contacting the ADPH Warehouse. Please reference form number 182. You also have the option of printing your own copies:

A companion document with descriptions of variables is available.

If you are an approved site provider, you can order condoms by contacting HIV Prevention. You must request access to HADIS through the ADPH Security Portal and once you are granted access, you must enter all required information onto the Condom Distribution form.
