Healthcare Providers

To Our Trusted Referring Partners,

Thank you for your continued efforts in referring patients to QuitNow Alabama to receive tobacco and nicotine cessation support. QuitNow Alabama has migrated to a new platform and therefore the way in which you refer has slightly changed. Moving forward, please follow the below updated instructions based on the type of referral you are hoping to place.

For Fax Referrals

  • Please utilize the updated fax form when sending in your referral.
  • Our fax number has also changed to 1-800-483-3114. Please make sure all referrals are sent to this new number.

For Referrals through the Online Portal

  • Follow these instructions to place your referral through the new Online Referral portal, located at https://rallycoaching.my.site.com/referral/s/.
  • When placing your first referral though the new portal, first search the NPI number under which your referring entity can be found and then follow the additional instructions as seen in the document. If you do not have any associated NPI number connected to your referring location, please contact the Quitline ([email protected]) to get registered. Once your registration is confirmed, you will be able to find your information listed in the portal to place your referrals under (following the instructions seen in the document).

For EHR/EMR Referrals

  • Email [email protected] to communicate with the RVO team to update the EMR referral or to add an EMR referral.

For additional information on the changes occurring to the Quitline through the platform transition, please see the Provider User Guide.

If you have any additional questions, do not hesitate to contact us at [email protected]. Thank you for your understanding and patience as this platform transition occurs.
