Effective October 1, 2024, the Center for Health Statistics will no longer accept in-person requests for regular certified copies of Alabama vital records. Requests for regular certified copies may be made in person at the Montgomery County Health Department or any other Alabama county health department. Visit Locations for more information. Apostille and Exemplified copies will continue to be processed at this location or may be requested by mail or by telephone.

In-person requests to change, correct, or register birth and death certificates will be accepted by APPOINTMENT ONLY. Please contact your assigned staff representative or call the Special Services Division at (334) 206-2637 to schedule an appointment.
