Payment Processing Information
For your convenience, the forms below have been created as fillable documents. Forms must be completed in their entirety and submitted to the State WIC Office in the format requested (mail or fax). All requests are subject to review and approval. Incomplete forms will not be processed; this includes forms missing additional documentation such as receipt/journal transaction.
Vendor Information Publication (VIP)
VIP is an e-newsletter published by the Vendor Management Branch for Alabama WIC vendors and clinics. The VIP contains important information regarding updates, compliance issues and policy reminders.
